We started Hera because we felt the system was letting down the generation that raised us. Hera is dedicated care management in the community for older adults, covered by Medicare. We’re depth over breadth and intend to be with families until end-of-life.

The role
- 1099, 30–40 hrs/week in business hours EST (you set your schedule)
- $40–60 / hour: factors include experience, geography, and client load
- Serve elderly adults with chronic conditions and their families: educate on navigating Medicare and the healthcare system, coordinate care across providers, advocate for what they need
You’d be a great fit if you:
- Hold an LMSW, LCSW, or RN (clinical license a plus but not required)
- Have experience in geriatric care management, hospice, palliative care, discharge planning, rehab, or hospital social work
- Love fostering connection with older adults and their families
- Are a creative problem solver who wants a job that looks different every day
- Are tech-savvy and excited by quickly evolving environments
- Remote - must be located in the United States
