Job Description:
- Serve as the first point of contact for new Nadia Care members
- Welcome families and conduct their critical first visit
- Educate members on Nadia Care services
- Complete initial intake and assess baseline needs
- Conduct up to 7 comprehensive virtual Welcome Visits per day
- Identify and escalate urgent medical or social needs to the clinical team
- Establish warm, trusting relationships with members
- Coordinate care transitions with Maternity Navigators and clinical specialists
- Pass critical intake notes to the core care team
- Log interactions, schedule visits, and track onboarding progress in systems such as Athena
- Enroll, schedule, and complete members’ first visits before handing them off for ongoing care
- Report directly to the Senior Manager of Care Delivery
Requirements:
- 3-5+ years in patient onboarding, member engagement, customer service, community health, or healthcare navigation
- Experience with Medicaid populations is a strong plus
- Fully fluent in English and Spanish, verbal and written
- Strong conversational fluency and comfort using non-technical terminology with members
- High emotional intelligence and active listening skills
- Ability to build immediate rapport while guiding members through structured talk tracks
- Comfortable navigating CRM platforms, virtual communication tools, and scheduling software such as Acuity, Spruce, EHRs, and MacOS
- Comfortable working independently in a fast-paced, high-growth startup environment
- Ability to hit daily outreach quality and engagement targets
- High school diploma or GED equivalent
